Insurance & Patient Co-Payments
We are contracted with most major dental insurance providers and are happy to help you understand your benefits. To ensure a smooth experience, please email or fax your dental and/or medical insurance information along with your New Patient Paperwork at least two business days before your appointment. This allows our team to verify your coverage and prepare a detailed benefits breakdown.
Be sure to bring your dental benefit card to your visit — our team will gladly explain your policy and help you maximize your insurance benefits.
If treatment is recommended, we’ll provide a detailed treatment plan outlining estimated insurance coverage and your out-of-pocket portion due at the time of service.
Please note: your insurance contract is between you and your provider. While our office will assist with claims and benefit coordination, final financial responsibility rests with the patient.

Appointments & Consultations
We welcome new patients and are happy to provide second-opinion consultations with Dr. Teah Nguyen.
For referred or self-referred patients, a comprehensive examination is required before a diagnosis or treatment plan is given. Please ensure
that x-rays and chart notes from any previous dental office
are transferred before your visit (note that many offices require 7–14
business days for record transfers).
If you are using dental or medical insurance, please provide all necessary information in advance so that we can create an accurate treatment estimate following your consultation.

If you are a patient to our office, the attached file contains our patient bundle with forms that will need to be filled out when you arrive at our office.
